In-home assessments will be completed to determine the needs of elders or individuals with a need for services.  The Operations’ Manager will meet with families to assess the needs of the individual and provide linkages of services and resources.

 

The in-home assessment will aid in determining eligibility for services such as:

  • Personal Care Services
  • Adult Day Care
  • Home Delivered Meals
  • Home Services
  • Senior Companion
  • Legacy Veterans Support
  • Home Chores
  • Transportation
  • Other Public Assistance

Assessments are required to be completed initially, prior to service authorization or service provision, for the services referenced above.  The assessment documents the client’s need for the requested service.  Reassessments must be conducted as needed based on changes in consumer’s status, usually more frequently than annually.  Clients who receive home delivered meals must be assessed every six months.
